Have you been diagnosed with Type 2 diabetes in the last 12 months?

Our Living Well, Taking Control programme lets you meet like-minded people in a relaxed setting to discuss your health needs and learn new tips to help you manage your wellbeing.

We deliver the courses in small groups that can help build your confidence and motivation to use your own skills. The facilitators provide health information and professional services to take effective control over your lifestyle; helping you lose weight, increase physical activity, eat well and understand your body. By following these simple tips you can reduce the risk for further complications and ease your symptoms.

Who is it for?

Individuals of all ages who have been diagnosed with Type 2 diabetes by their GP in the last 12 months. If you are unsure if you qualify call our team 0121 663 0007.

Where is it located?

The Living Well, Taking Control: Type 2 diabetes management service is currently available to people with a GP in South Birmingham practices. If you are unsure if you qualify call our team 0121 663 0007.

To enrol today, ask your GP or practice nurse for a referral.
