Preventing Diabetes

Are you at risk of developing Type 2 diabetes? We can give you tips on managing your lifestyle to prevent early onset of the condition.

The Healthier You: NHS Diabetes Prevention Programme, delivered by Living Well Taking Control, lets you meet like-minded people in a relaxed setting to discuss your health needs and learn new tips to help you manage your wellbeing.

Our service is delivered in small groups that can help build your confidence and motivation to use your own skills. The facilitators provide health information and professional services to take effective control over your lifestyle helping you lose weight, increase physical activity, eat well and understand your body.

Who is it for?

Individuals of all ages with a HbA1c (blood sugar) reading of 42-47m/mmol, who are classified as being at high risk of developing Type 2 diabetes.

If you don’t know your HbA1c reading you can take the online risk assessment from Diabetes UK to calculate your risk. If the assessment tells you that you are at high risk, you should contact your GP for a blood test to find out more.

Where is it located?

The Living Well, Taking Control: Preventing Diabetes service is currently available in Birmingham, Bristol, Bury, Cornwall, Dudley, Durham, Easington and Sedgefield, Gloucestershire, Heywood, Middleton and Rochdale, Isles of Scilly, North Somerset, Sandwell, Solihull, South Gloucestershire, Southport and Formby, Sefton, St Helens, Walsall and Wolverhampton.
